OTOPLASTY

Otoplasty is a cosmetic surgery technique that corrects the size, angle, and shape of the auricle, i.e., the defect most commonly known as “protruding ears.” In addition, it is possible to shape and modify other details of the pinna (outer ear) and lobes, any asymmetries due to congenital defects (already present at birth), or problems acquired as a result of trauma.

PREPARATION FOR OTOPLASTY SURGERY

Before otoplasty surgery, all blood tests and ECGs listed and planned at the first visit should be performed. In all clinical cases, custom-made sheaths or “tennis bands” made for each patient are designed. Numerous pre-operative images will be taken for the planning of an increasingly personalized therapeutic course aimed at visualizing well the different angulation of the two ears and their structural morphology (in fact, it is well known that almost no part of the body is in perfect symmetry with respect to the contralateral; studying in detail these small differences increasingly lead the Surgeon to perfection of result).

Any current drug therapies (especially with cortisone, contraceptives, antihypertensives, cardioactive agents, anticoagulants, hypoglycemic agents, antibiotics, tranquilizers, sleeping pills, and excitants), homeopathic and phytopathic therapies, and possible allergies to antibiotics and drugs in general should be reported. At least two weeks prior to otoplasty surgery, one should stop taking medications containing acetylsalicylic acid (such as Aspirin, Cemirit, Vivin C, Ascriptin, Bufferin, Alka-Seltzer, etc…). Regarding the possible intake of substances such as low-molecular-weight heparin, possible discontinuation should be properly discussed with the Surgeon.

At least 2 weeks beforehand it is recommended to discontinue smoking, which adversely affects the healing processes.

The day before otoplasty surgery or even better, the morning itself, a thorough hair washing should be performed, nail polish should be removed from the fingernails, and cosmetics from the face should be removed.

INFORMED CONSENT
When the patient has clarified all doubts and decides to undergo otoplasty surgery, he or she is guided in the completion of the informed consent document, drawn up according to his or her clinical case.

CLINICAL ASSESSMENTS

The patient must perform the clinical examinations prescribed by the Surgeon, before undergoing otoplasty surgery, to certify and demonstrate the suitability of the health status for the total safety and peace of mind of the patient, which must always be the first concern. In case any pathologies are discovered or the examinations record abnormalities, it will be the Surgeon’s responsibility to inform about the state of health and to be able to plan the otoplasty surgery with other anesthesiological techniques to avoid any risk related to them.

ADMISSION OF THE PATIENT

The patient is admitted in the morning on an empty stomach from midnight the night before. ANESTHESIA
Dr. Mattia Colli has decided to abandon full anesthesia with its associated dangers: surgeries are performed with the most modern anesthesiological techniques of deep sedation (local anesthesia + intravenous anesthesia), so that fear and pain sensation are completely cancelled while the unconscious is invaded by pleasant dreams and images.

Local anesthesia occurs through the introduction of anesthetic into the areas of the earlobe to be treated in order to effectively eliminate the sensation of pain. The insertion of this solution, called tumescent, also takes place with a vasoconstrictor (adrenaline) to avoid bleeding, because it reduces the blood flow in the area to be treated allowing the operating surgeon to work at his best and in a delicate way, managing to eliminate a greater amount of fat.

Another advantage is to have an immediate return to social life without having to endure classic situations of fatigue, dizziness and exhaustion typical of the disposal of general anesthesia drugs.

OTOPLASTY: SURGERY

 

The Surgeon, during otoplasty surgery, proceeds with the incision in the posterior area of the ear; after removing the tissues he accesses the auricular cartilage and makes a thin incision without removing or eliminating the noble tissue. The cartilage is preserved and “unbent,” that is, bent so that the correct fold is assumed so as to reduce the protrusion of the ear.

The new angle assumed by the cartilage fold is then fixed with special suture threads covered with a silicone weave that allow permanent stability, reduce the risk of infection, and allow faster healing.

After the otoplasty operation is completed, the top of the head is dressed with an elastic bandage for the purpose of compressing and protecting the treated area from impact and preventing swelling from altering the surgical result achieved. The special dressing promotes healing of the pinnae according to the optimal shape.

Otoplasty surgery does not require hospitalization, and the patient can safely return home after a few hours.

OTOPLASTICS
POST-OPERATIVE

 

The dressing is removed after a few days in order to optimally disinfect and clean the wound. Following otoplasty surgery, the Surgeon suggests a series of practices to be maintained to promote healing and complete healing; the bandage and dressing past the first hours of the postoperative course of otoplasty surgery is relaxed and “tennis bands” are provided in order to secure the dressings around the ears.

After the dressings are removed, it is normal to see the treated anatomical area swollen and edematous. Otoplasty surgery does not involve pain; in fact, again after assessing the personal pain threshold according to the individual patient, most patients report taking no pain medication during recovery. All medications and related antibiotic and anti-inflammatory prophylaxis prescriptions are provided at the end of the otoplasty surgery before the patient leaves the clinic.

Work can be safely resumed 48h after the otoplasty surgery, although the patient will keep the bandage with the corresponding dressing for about a week; preferably, children should abstain from school one week, a time of safety to avoid accidental blows or that may compromise the aesthetic outcome of the otoplasty.

For a few days after otoplasty surgery, the dressing should be maintained, which is intended to protect the operated pinnae, limit their swelling and the sensation of discomfort. In addition, in the first 2 to 4 days after otoplasty surgery, it is advisable to stay in cool rooms and keep the head elevated by 2 or 3 pillows during bed rest to limit swelling. The use of cold compresses on the face may help to provide a cooling sensation and reduce pain and swelling.

The result is to be considered permanent, but in many patients the molded cartilage tends to return to its previous position; to avoid this unpleasantness, the stitches will be removed only after 14 days and the band will be worn at night for at least 1 month. After this period has passed, the patient is out of any danger of this possibility.

RESULT
Otoplasty surgery allows the correction and permanent shaping of protruding ears, reducing the excessive protrusion of the ears by bringing them closer to the mastoid (head).
